We offer a nurturing, safe environment designed for you or your loved one who wish to maintain their independence yet still require some level of assistance. Our adult family home provides a variety of care and support services with qualified staff present around the clock.
Our services are designed to help our residents become more engaged, social, and active than they would be if they were living alone. We offer an alternative to nursing home and in-home care and provide an atmosphere which allows our residents to feel like they are living in their own home. The family and friends of our residents have our assurance that their loved one will be well taken care of and will be treated with the utmost dignity and respect. We treat our residents as if they are our own family.
Care Services Provided
4 Elements AFH combines the comfort of a homelike setting with the security of direct access to quality support services and trained healthcare professionals. Our care services include:
- Assistance with Activities of Daily Living (ADLs)
We offer personal care and assistance with activities of daily living, including bathing, grooming, dressing, toileting, shaving, oral hygiene, and more. - Dementia and Mental Health Care
We have qualified mental health care professionals who can offer the necessary support that residents with dementia and mental health issues need. - Parkinson’s Care
We offer 24/7 supervision, housekeeping, personal care assistance, meal preparation, and medication monitoring, all of which can greatly benefit our residents with Parkinson’s disease. - Pharmacy and Med Delivery
With our pharmacy and med delivery service, we make sure that our residents observe proper medication adherence, taking the right medication with the right dose and at the right time. - Private and Semi-Private Rooms
We offer comfortable private and semi-private rooms to ensure the privacy of each resident. Rooms can be furnished with your own belongings or ours. - All Meals
We prepare and serve delicious, healthy meals and snacks throughout the day. Meals are prepared according to each resident’s preferences and dietary requirements. - Special Diets
We create specific meal plans to meet the special dietary requirements of our residents. We can prepare diabetes-friendly meals, vegetarian meals, and more. - Laundry
It may seem like a minor thing, but having clean clothes is important to one’s health as well. To ensure our residents have clean clothes every day, we take care of all laundry tasks. - Podiatry Care (On Site Visits)
We offer regular on-site visits from licensed podiatrists who can assess and treat a range of foot and ankle conditions, including diabetes complications, bunions, ingrown toenails, callouses, and more. - PT and OT (On Site Visits)
We schedule regular on-site visits from qualified physical and occupational therapists to ensure our residents can perform activities of daily living and that they are in physically good shape. - In-Home MD/ANRP Visits
We offer in-home MD/ANRP visits to clients who wish to be assessed in their own residence. They can evaluate, treat, and make recommendations to ensure that clients are getting the best care possible. - Wound Care (On Site Visits)
As needed wound care specialists are available to assess and treat all types of wounds from pressure ulcers, surgical wounds, minor cuts, lacerations, and more.
